GDPR (General Data Protection Regulation) May 2018
All organisations that process personal information (names, contact details, etc.) have to comply - even small voluntary associations. Under the regulations, organisations need to:
Explain why they collect personal data and how long they retain it
Only use it for that purpose or others the person consents to
Gain positive consent to collect, store and process personal data
Gain a separate consent if appropriate for each medium they wish to use to communicate (e-mail, phone, post)
Make it easy for people to find out what personal data they store on them and change or delete it.
This form is designed to facilitate compliance by collecting consents
